Convenient DOT Physical Appointments to Save You Time!

DOT Physicals cost $100 (employee pays at time of appointment, or we can invoice the employer. Please let your employee inform us of how they are going to pay at time of appointment)

If you are a commercial driver for the Department of Transportation (DOT), keeping your DOT medical card up to date is essential. One requirement of this process is getting a DOT physical. At ARCpoint Labs of St. Charles, we have partnered with Dr. Glenn Ezell of Ezell Chiropractic to keep you on the road! We understand that finding the time to fulfill this important mandate can be challenging. That’s why we go above and beyond to make the process smooth and seamless, providing comprehensive DOT solutions to ensure compliance with federal rules and regulations.

DOT PHYSICALS ARE ONLY OFFERED ON TUESDAY AND FRIDAY MORNING, if an appointment is made past 12:00pm we will call to reschedule!

PLEASE ARRIVE 5-10 MINUTES BEFORE YOUR APPOINTMENT!

PLEASE COMPLETE THE MEDICAL HISTORY FORM THAT WILL BE SENT TO YOU PRIOR TO YOUR APPOINTMENT!

ITEMS TO BRING:

• Your current state-issued driver's license
• Glasses, contacts, hearing aids (if applicable)
• List of all medications that you are currently taking
• Hemoglobin A1C levels for diabetics, done within 3 months
• CPAP compliance log - 3 months worth for sleep apnea
• Clearance letters for cardiac conditions other than blood pressure

THINGS TO REMEMBER:

• Make sure to be well hydrated as we will need a urine sample to test for things like kidney function, sugar, and others. THIS IS NOT A DRUG TEST. If you require a drug test with your physical, we can complete that as well. Just come prepared with your drug testing chain of custody form. 
• Do not smoke or drink caffeine before your exam or your blood pressure may rise
• Drivers are typically in and out in under 30 minutes
• Drivers are awarded a certification that can last up to a maximum of 2 years
• Some drivers may require other testing by the doctor or another specialist
